Update Personal Information

With Patient Portal, keeping your personal information current is easier than ever. Simply click the account tab on the left hand side and edit your mailing address, emergency contacts, pharmacy recommendations, and much more.

View Medical Records

While logged into your Patient Portal, you can easily view and print your medical records, lab results, and doctor referrals. No more waiting on the clinic to respond to your voicemails, just log on and view instantly.

Request Prescription Refills

Now you can quickly and easily request your refills without making a visit to the clinic. Just log into your Portal and fill out a prescription refill request under the "messages" tab.

Appointment Requests

Patient Portal allows our clients to efficiently submit appointment requests online. Clients can choose between several different appointment types, as well as provide multiple time preferences. After a request is submitted, one of our team members reviews and responds via phone or email. After confirmation is received from the client, the appointment is considered booked.
